Making a Referral to Gentleway Community Care
 

We welcome referrals for adults (18+) who require safe, effective, person‑centred support in their own homes. Our referral process is designed to be transparent, responsive, and aligned with CQC expectations, ensuring every person receives the right support at the right time.

Who Can Make a Referral 

We accept referral from:

  • Local authority commissioning teams
  • Social workers and care managers
  • NHS professionals
  • Case managers and multidisciplinary teams
  • Families, carers, and advocates

 

 

 

 

 

We work collaboratively with all professionals involved to ensure a safe and well‑coordinated transition into our service.

Our CQC-Aligned Referral Process 

Our referral pathway reflects the CQC’s focus on safety, responsiveness, and effective assessment. All information shared with us during the referral process is handled confidentially and in line with GDPR requirements.

 

1. Initial enquiry

  • We gather essential information to understand the person’s needs, risks, and desired outcomes. This ensures we respond promptly and appropriately

 

2. Needs assessment

 

A detailed assessment is completed to identify:

  • Support needs
  • Strengths and goals
  • Communication preferences
  • Environmental considerations

 

3. Risk assessment

We complete a thorough risk assessment covering:

 

  • Health and wellbeing
  • Behavioural risks
  • Environmental risks
  • Safeguarding considerations

This ensures the service is Safe and appropriate before support begins.

 

4. Support planning

 

We develop a personalised support plan that is:

  • Person‑centred
  • Strengths‑based
  • Outcome‑focused
  • Co‑produced with the individual and professionals

This aligns with Caring, Effective, and Responsive care.

 

5. Transition planning

 

Where required, we create a structured transition plan to ensure:

  • Consistency
  • Clear communication
  • Reduced anxiety
  • Safe handover from previous services

This demonstrates Well‑Led and Responsive practice.

 

6. Start of service

 

Support begins once all assessments, risk plans, and agreements are in place. We monitor the first weeks closely to ensure the service is safe, effective, and meeting the person’s needs.
